§ 19-3-104. ISSUANCE OF PERMIT.

(a)

Issuance. The Department shall issue a permit to each applicant who meets the requirements of this title.

(b)

Contents. The Department shall include on each permit that the Department issues:

(1)

The name, address, and business telephone number of the person making the application;

(2)

The number of the permit;

(3)

The date issued and the expiration date;

(4)

The conditions and limitations applicable to the permit; and

(5)

Any other information determined by the Department.
