§ 21-18-103. APPLICATION FOR A PERMIT.

At least 15 days before the time when the permit applied for is to become effective, an applicant shall file with the Department an application which shall contain the following information:

(1)

The name and address of the employing or sponsoring person, agency, or entity that will solicit;

(2)

The exact location or locations where each solicitor will be assigned;

(3)

The time frame and duration of the solicitation;

(4)

The means of travel to and from the place of solicitation; and

(5)

The name, address, and telephone number of a contact person who will be able to provide additional information to the Department or its designee.
