§ 13-4-404. REINSTATEMENT OF A SUSPENDED PERMIT.

  • (a)

    Request for reinstatement. A person whose permit has been suspended may apply at any time for a reinspection for the purpose of reinstatement of the permit.

    (b)

    Contents of the application. An application for reinspection shall include a statement signed by the applicant that in the applicant's opinion the conditions causing suspension of the permit have been corrected.

    (c)

    Inspection. The approving authority shall make a reinspection of the facility within 10 days after receiving a written request under this section.

    (d)

    Reinstatement. The approving authority shall reinstate the permit if the approving authority finds that the applicant is in compliance with:

    (1)

    The requirements of the Code and state law;

    (2)

    County regulations;

    (3)

    Any order issued by the approving authority;

    (4)

    The requirements of the permit; or

    (5)

    The requirements of a correction notice or citation issued under Article 3, Title 6 of the Code.
